2 Prepare information
- Provide as much detail as possible: what happened, when, where, who was involved (if known), any supporting evidence.
- You may choose to remain anonymous, or provide your identity if you wish.
3 Submit your report
4 Receive your access code
- After submitting, you are normally given a unique report code or number so you can follow up later.
5 Follow up on your report
- Log in to the platform using your code to see status updates, respond to requests for further information, or upload additional evidence.

- Violations of the Code of Ethics
- Moral, sexual, or any form of harassment
- Conflicts of interest
- Financial fraud, theft, or embezzlement
- Misuse of budgets
- Unauthorized disclosure of confidential information
